To Request a Copy of Your Medical Records

Contact the Medical Records Department at 914.666.1855 during the following business hours:
Monday to Friday, 7:00am-7:00pm
Please note the office is closed weekends and holidays.

These documents are required before we can release your medical records.  Your request will be processed within 5 business days from the date we receive these items:

• a completed Authorization Form 
• a copy of photo identification

The Authorization Form and the photo identification can be faxed to 914.666.1013
or mailed to:  NWH Medical Records Department, 400 East Main Street, Mount Kisco NY 10549.
