Your Medical Records
To help you get your medical records when you want them, there are two ways you can access them:
- View online through The NWH Patient Portal
- Request copies of your records in theMedical Records Office
1. Go to The NWH Patient Portal
To set up the NWH Patient Portal you need your Patient Code. Your Patient Code was given to you when you were in the Hospital. If you do not have your Patient Code or your Code expired, please call 914.242.7944.
The NWH Patient Portal works best with the right browser: Internet Explorer 9 (or above), Firefox, Mozilla, Chrome, and Safari. Internet Explorer 8 is supported only through ‘Compatibility Mode’. Accessing the portal through any other browser is not supported and may lead to loss of functionality.
By setting up the NWH Patient Portal you can conveniently and securely view selected health information from your stay at NWH, online anytime.

2. Request Records from the Medical Records Department
Contact the Medical Records Department during the following business hours:
Monday to Friday, 7:00am-7:00pm
Please note the office is closed weekends and holidays
These documents are required before we can release your medical records.
Your request will be processed within 5 business days from the date we receive these items:
- a completed Authorization Form
- a copy of photo identification
This Authorization Form and the photo identification can be faxed to 914.666.1013
or mailed to: NWH Medical Records Department, 400 East Main Street, Mount Kisco NY 10549.
